<p>Heute werden deutschlandweit über 2000 Covid-Patienten intensivmedizinisch betreut. <p>Wenn aber Corona irgendwann einmal Geschichte ist, wird es einen Neustart geben müssen in der Intensivmedizin. Allerspätestens dann. Denn die Lage ist ernst.<span id="more-1852"></span></p>
<p>Was brauchen wir dann?</p>
<p>&nbsp;</p>
<p><strong>1. Kassensturz. Schonungslose Bestandsaufnahme:</strong></p>
<p>Wer ist eigentlich noch da? Wer bleibt?</p>
<p>Schon jetzt haben in einer Umfrage des Deutschen Krankenhausinstituts 72 Prozent der befragten Krankenhäuser angegeben, weniger Intensivpflegepersonal zur Verfügung zu haben, als noch Ende 2020. Ich fürchte, dass noch mehr gehen werden.</p>
<p>Ein großer Teil hat aus Loyalität zu Patient, Kollegium und Gesellschaft gesagt: „Ich halte durch. Aber: Wenn die Pandemie vorbei ist, suche ich mir etwas anderes. Too much.“ Ich hoffe, es bleiben möglichst viele in diesem tollen Beruf. Aber verdenken werde ich es niemandem, der sagt: „So long, farewell, auf Wiedersehen. Goodbye.“</p>
<p>Ich hoffe, wir können möglichst viele zum Bleiben bewegen, denn der Arbeitsbereich Intensivstation ist faszinierend.</p>
<p>&nbsp;</p>
<p><strong>2. Barmherzigkeit mit dem Gegenüber</strong></p>
<p>Wir sind alle total durch. Wir haben mehr gearbeitet denn je. Wir haben viel Leid gesehen und Sterben bezeugt. Wir haben plötzliche Todesfälle von jungen Menschen miterlebt. Bei jeder Auseinandersetzung mit Kollegen, Angehörigen, Patienten und MitarbeiterInnen anderer  Professionen müssen wir das im Kopf behalten. Wir alle tragen ein schweres Bündel dieser Last, unverarbeiteter Erlebnisse. Ich merke an meinem eigenen Nervenkostüm, das bei vielen Reaktionen und auch dieses Päckchen eine große Rolle spielt. Bei jeder „merkwürdigen“ Reaktion des Gegenübers sollten wir vor einer Reaktion dies bedenken: Er oder sie hat zwei Jahre Pandemie in den Knochen und sicher einen harten Tag. Wir brauchen nun -mehr denn je- Nachsicht miteinander. Nicht alles ist erlaubt, aber manches muss man vielleicht erst am Folgetag besprechen, wenn die Wogen etwas geglättet sind. „Versöhnen statt spalten“ hat Johannes Rau das genannt.</p>
<p>&nbsp;</p>
<p><strong>3. Leadership und Aufbruch</strong></p>
<p>Wir brauchen jetzt keine Unternehmensberater, die uns erzählen, wie wir die Liegezeit verkürzen oder mehr Income generieren können (Hab solche Menschen noch nicht kennengelernt, aber bin auch nicht heiß drauf).</p>
<p>Wir brauchen die Menschen auf den Stationen, den Wachen und den Notaufnahmen, die bereit sind, zu analysieren, was jetzt dran ist. Was wir besser machen können. Auf welche Tätigkeiten wir vielleicht auch verzichten können, um die angestiegene Workload bei immer weniger Intensivkräften zu beherrschen.</p>
<p>Diese Mitarbeiterinnen und Mitarbeiter müssen wir um jeden Preis motiviert halten. Wir müssen sie von sinnbefreiten Tätigkeiten, übermäßigen Dokumentationspflichten und unnötigen Lasten, die sie tragen müssen, befreien. Wir sollten sie so befähigen, dass so zu tun, was sie am besten können: Intensivmedizin. Patientenversorgung. Notfallversorgung.</p>
<p>Sie werden uns sagen, was sie brauchen, um ordentlich zu arbeiten. Wir sollten auf sie hören und sie machen lassen. Diese Menschen haben die Pandemie gewuppt in den Krankenhäusern. Sie haben gezeigt, dass sie es können. Es geht aber immer besser. Dafür braucht es bessere Bedingungen.</p>
<p>Wir haben so vieles erreicht und gelernt in der Zeit. Wir haben Abläufe verändert und Neues geschaffen. Wir brauchen nun die, die bereit sind, die Ärmel hochzukrempeln und den Laden in Schwung zu bringen.</p>
<p>Freue mich über jeden, der nun bereit ist, es gemeinsam besser zu machen. Es ist ein toller Beruf. <div>Life is busy the way it is, and if we don’t set goals for ourselves and stick to them and work on them every day, somebody else will determine our schedule.</div>
<div></div>
<p>Nobody ever learned a skill or wrote publications or finished a grant proposal by accident. That won’t happen!</p>
<p>Forget new years resolutions, they are vague and won’t get you anywhere. If you want to be successful you have to set smart goals and work on them daily.</p>

<div></div>
<div><strong>S.M.AR.T. goals</strong> have five characteristics:</div>
<div></div>
<div>They are&#8230;</div>
<ol>
<li><strong>Specific</strong>:<br />
&#8220;Become a better human being&#8221; is kind of vague.</li>
<li><strong>Measurable</strong>:<br />
How will you know you have reached it, if you can’t measure it? &#8220;Finish three internships</li>
<li><strong>Attainable and</strong></li>
<li><strong>Realistic</strong><br />
Your goals should be ambitious so that you are pushed to your limits. But also they should be realistic. &#8220;Nobel prize 2015&#8221; could be difficult.</li>
<li><strong>Time-bound:</strong><br />
To be and stay motivated you have to reach a goal from time to time. It should be possible to reach it soon. If you have one big goal you should break it down in smaller goals.</li>
</ol>
<div>I personally set goals that can be reached in three to six months.</div>

<div>Here are some examples on bad and good goals.</div>
<div></div>
<p><strong>Bad goal:</strong> I want to learn ultrasound.<br />
<strong>Good goal:</strong> I will finish an online sonography course until January 31st. I will look over the shoulder of Dr. Miller (ultrasound god) in my free time once a week until march. Until June I will have conducted 200 sonograms myself.</p>
<p><strong>Bad goal:</strong> I want to finish the case report on the patient, who came in with diarrhea and a potassium of 1,3 mol/l.<br />
<strong>Good goal:</strong> I will hand over the case report to Dr. Chandler (boss) in three weeks. That means I have to get the patient’s chart today, finish the introduction by friday, the discussion by next Wednesday and the charts by next friday.</p>

<p><strong>Bad goal:</strong> I want to finish my doctor thesis by the end of my studies (e.g.2023).  This is not time-bound. This goal has to be breaker down in smaller goals<br />
<strong>Good goal: </strong></p>
<ul>
<li>I will have at least monthly meetings with my mentor to talk about the progress of my work.</li>
<li>In the first meeting I will discuss which publications should be discussed in the introduction and the discussion part.</li>
<li>I will finish the Introduction by March and the methods part will be kept uptodate, meaning that all methods that I use will be inserted in the methods part in &#8220;real-time&#8221;.</li>
<li>Prepare the charts and graphics as soon as I finish the lab work.</li>
<li>I will read one relevant publication a week.</li>
</ul>

<p><strong>Bad goal:</strong> I want to be a better communicator with the nurses.</p>
<div><strong>Good goal:</strong> I will meet with the responsible nurse once at the beginning and once on the end of my shift and talk my patients through with him.</div>
<div></div>
<p>Once you have decided on your goals there is one important thing to do:</p>
<div></div>
<p>It is really important that you write them down, otherwise you will easily forget them. So write them down and keep them in a place where you can see them. Your wallet, your desk, your bed. Review them constantly at least weekly.</p>
<p>Success doesn’t happen by coincidence. You have to plan for your goals to be reached, they won’t happen accidentally.</p>
